Serious anaerobic infection — adults: 7.5 mg/kg every 6 h

Maintenance — 7.5 mg/kg IV every 6 h

Anaerobic infection — adults: 800 mg then 400 mg every 8 h

Anaerobic infection — 8 weeks–12 y: 7.5 mg/kg every 8 h
